Controlled Shipping Level 1 and Level 2 Exit Letter
Instructions
The attached template is used to notify suppliers that they are approved to exit Controlled Shipping – Level 1 or Controlled Shipping – Level 2.
The SQE issues the CS1 exit. The CS2 coordinator team issues all CS2 exits unless they are
outside of North America and the SQE issues the exit.
Enter identification information specific to the supplier that is being released from Controlled
Shipping – Level 1 or Controlled Shipping – Level 2.
Enter the part numbers of all impacted part numbers.
Enter the part name or description of all impacted part numbers.
List the defects as noted on the PRR.
List all PRR numbers related to the non-conformity.
List all customer plant or operations affected.
Listed the Vehicle Lines or Product Lines affected.
Note any special instructions given to the supplier.
Place an “X” or a tick mark next to the appropriate disposition.
o The first three (3) of the six (6) selections should be utilized when exiting from Controlled Shipping Level 2.
o The second three (3) of the six (6) selections should be utilized when exiting from Controlled Shipping Level 1.
o The last box is when this CS2 was issued in error and you want to cancel CS1 or CS2.
Enter the name, title and provide the signature of the person issuing the exit letter.

Your facility has met all Exit criteria to be removed from both Controlled Shipping-Level 2 and Controlled Shipping-Level 1. These parts will be monitored periodically by GM Supplier Quality
Assurance and continuously by our assembly plants. Any degradation in quality will not be tolerated. Your facility has met the Exit criteria to be removed from the Controlled Shipping-Level 2. Based on discussion between our organizations, this/these part/s will remain in Controlled Shipping – Level 1 until all exit criteria are met.

Your facility has met all Exit criteria to be removed from Controlled Shipping-Level 1. These parts will be monitored periodically by GM Supplier Quality Assurance and continuously by our assembly plants. Any degradation in quality will not be tolerated. All Controlled Shipping requirements have been cancelled. All Controlled Shipping records specific to this issue will be deleted immediately from the Global Quality Tracking System (GQTS).
